To Register

 

Thank you for your interest in attending PeakCare's Annual General Meeting to be held at 4.30pm on 6 December at Riverside Receptions, 50 Oxlade Drive, New Farm followed by our end-of-year celebrations and PeakCare's traditional Trivia with a Twist competition.

When submitting your registration, you will be asked to indicate whether you are registering for:

  1. The AGM only - no fee will be charged if you are attending the AGM only
  2. The AGM plus end-of-year celebrations including a light supper – for a fee of $45 plus GST
  3. The AGM plus end-of-year celebrations and the Trivia with a Twist competition – stay for the Trivia with a Twist competition and receive a $10 discount (i.e. $35 plus GST)

Please enter the following information:

The name of your organisation in full (eg. UnitingCare Queensland; Mercy Community Services; Churches of Christ Care; Queensland Police Service)



If you have selected B (AGM plus end-of-year celebrations) or C (AGM plus end-of-year celebrations and the Trivia with a Twist competition), to complete your registration you must also enter the name and details of the person to whom an invoice and options for paying your contribution to the catering costs for this event are to be sent.

 

If you have selected A (AGM only), please enter N/A in each of the fields below.

 

Please note that no refund or discount of a due payment will be made if written notice of a cancellation of your registration is not emailed to groberts@peakcare.org.au at least seven days prior to the event.

After submitting your registration, you will receive an email confirming your registration. If you do not receive this email within 10 minutes, it is likely that you have incorrectly entered your email address and will need to repeat the registration process.
